Who We Are
Community Connections Team is a program that goes beyond the walls of the health care clinic and helps patients connect with community resources to meet their basic needs. We know by addressing these basic needs outside of the clinic walls has positive downstream health outcome implications.
What We Do
Screen patients for 12 social determinants of health to identify individual or family needs. Follow up with patients to refer to local community resources to alleviate their need(s).
